We hope you will fill in as much of the enclosed form as you feel able to.

Personal details about the children are only needed for our sponsors statistics of age and gender of the children, including siblings.

Diagnosis is useful to us in sending relevant information to you from time to time.
All the information you give us is completely confidential.

The benefits of becoming a member will enable you to:

• Borrow books from the library, (Free of charge).
• Have priority booking at events where spaces are limited.
• Vote at the AGM
• Receive our regular emails about events, speakers etc

We make a nominal charge of £5 per Family, per year, starting at the beginning of April.
Cheques should be made payable to “SPACE Support Chesham” and sent, with your application form to:

or cash can be brought to the next Support Group.

We would like people to feel that they are part of a community of parents who support each other. Everyone is valued and special and have experiences that they can share.

Even if you can’t get to any of our meetings at present, we would like to think that we can bring hope and encouragement and are very happy to keep in touch with you.
